- Health Forms
All new students are required to complete a health form. You must see a doctor in order to fill out sections of this form, so be sure to make an appointment as soon as possible, as it can sometimes take several weeks or even a month to get an appointment.
o Health Insurance Information
Moore College of Art & Design requires that all students have health insurance. If you are insured under a health plan, you will need to submit documentation of coverage and waive out of the plan offered by the College. Health insurance information, costs, and the link to the waiver system will be made available via Moodle on the New Student Guide. Students without coverage will need to purchase Moore’s insurance (or a comparable plan). Note, if you do not have adequate coverage, or do not waive out, you will be automatically enrolled and billed for the insurance plan.

Housing Applications are now being accepted for the 2017-2018 school year and can be found at the link below. The Priority Housing Deadline is June 1st, 2017. o Foundation Art Kit (Details here)
The Foundation Department faculty worked alongside representatives from all majors to create the “Foundation Supply Kit”. These kits are designed as a starting block for which you will be able to build the rest of your supplies throughout your 4 years at Moore. The College has worked diligently with Blick Art Supplies to get you the best deal possible with the highest quality materials at the lowest cost. This year, Foundation anticipates the cost of the kit to be around $465 (which is less than $70 per studio course). While the initial cost may seem high, remember that these supplies are sold to you at a great discount (up to 50% off retail value) and will give you the materials you will need to successfully complete your first year of studio courses. Buying these same items out of pocket would almost double the cost of your supplies.
Please note that this is a required purchase and should be budgeted as a necessary college expense. Representatives will be on-campus the first day of Orientation to distribute these kits.
