Request a transcript

Requests must be made in writing using the request form below or in a letter. If you write a letter, it must include the following:

• The student's name as it was while attending if different from now
• Moore ID# or Social Security Number
• Dates attended Moore
• Status: Enrolled, Graduated or Withdrawn
• Address to where the transcript should be sent
• Ten dollar charge for each copy. Payments by Check or Money Order must be mailed; Payments made with a Credit Card must be prepaid by contacting the Business Office at 215.965.4098. If you pay by credit card you may fax your request to 215.965.8538.
• Telephone number in the event we have to make contact
• Letters must be signed
